Understanding chronic back pain

Persistent back pain is rarely as simple as a single structure being damaged or out of place.

While injuries, degenerative changes and physical restrictions can all contribute to symptoms, they are often only part of the picture.

Many people are surprised to learn that scans do not always explain the amount of pain someone is experiencing. In fact, it is common for people with significant changes on MRI scans to have very little pain, while others experience considerable discomfort despite relatively normal findings.

Pain is always real, but it does not always tell us exactly what is causing it.

When back pain persists, I believe it is important to look beyond the area that hurts.

Symptoms often develop in layers over time. Previous injuries, surgery, scars, stress, trauma, movement habits, life experiences and general health can all contribute to the overall picture. These layers are often closely connected, influencing and reinforcing one another.

Sometimes back pain is primarily driven by physical factors. Sometimes the nervous system has become more protective and sensitive over time. More often, it is a combination of influences working together.

Part of my role is to help untangle those layers and understand which factors may be most relevant for you.

One of the things I often tell patients is that nothing is irrelevant.

A previous injury, an old scar, a stressful life event, a difficult pregnancy, surgery years ago, poor sleep or changes in activity levels may all provide useful clues about why symptoms are behaving the way they are.

Taking the time to understand your story helps build a more complete picture of what may be contributing to your pain.

Assessment begins by listening.

I want to understand not only where your pain is, but how it developed, how it affects your life and what you think may be contributing to it.

I don’t believe anyone has all the answers when it comes to persistent pain.

What I can offer is time, careful listening, clinical expertise and a willingness to look beyond obvious explanations.
